Attach your responses to the questionnaire;
Review the consent portion, and then sign to have your medical records released.
OPTION 2: USE OUR ONLINE BOOKING SYSTEM

Please inform us when checking in, and wear a mask if you are experiencing any of these symptoms: Fever (37.8°C or greater), any new or worsening respiratory symptoms, any new onset of atypical symptoms including but not limited to chills, muscle aches, diarrhea, malaise, or headache.
